3 Celebration of Life/Memorial Service proposal

We are alongside you as you move through this time.

When you are ready, please take a moment to offer your vision for how you wish to celebrate your loved one's life. If you discover that you need support navigating the form, please call me at the number below and I will be happy to guide you.

After you have shared your vision, Rev. David or myself will be in touch with you to learn more about its details. If you have chosen to receive Pastoral Care, the respective party will also be in contact via your preferred indicated method.

We would be honored to support you and hold sacred space as you share the beauty of your loved one's life.

Choose the type of service options you are interested in. Livestreaming makes the service available to your loved ones who cannot attend. It will be streamed via Zoom.

In-Person Service - No Livestreaming

In-Person with Livestreaming (Livestreaming available in the Sanctuary or Friendship only, may require tech team)

Celebration of Life Guidance Agreement *

Please check the box below indicating you have read and agree to the following. A copy of this form will be emailed to you upon agreement:

Over the next few weeks, I invite you to consider some photos, memorabilia, or other items you’d like to share during your loved one’s celebration of life.

Your officiant will support you with:

Planning the celebration of life service.
Creating an “order of service” to include what type of music you wish to have in the COL service, and when/if you wish to have a slideshow played.
Any special words, quotes, readings, or scripture you would like to have integrated into the service on your loved one’s behalf.
Whether you wish to have people share freely or a select few throughout the COL.

Anaiya will support you with:

Planning and prepping space, time, and date.
Receiving deliveries the day before or day of the COL.
Arranging Friendship Hall before COL for the reception if you have chosen to have one on campus.
Placing photos, memorabilia, and/ or any decor the family chooses.
Coordinating sound/audio technicians if needed.
Placing out food, making coffee, and supporting the family.

Things to bring with you the day BEFORE the Celebration of Life

Framed photos of your loved one, sign-in book if you choose to have one, decorations, food.
Service program if you choose to have one made and two family/friend ushers to distribute them.
Any other mementos that are important for you to have and share with your guests or loved ones.

Media Content and Digital Assets

Please ensure all media files are either mp4, png., jpg.
Please ensure all digital assets are delivered via flash drive or email three days prior to the COL.
If you choose to have a slideshow, you or someone you know will need to create the content and deliver it to Anaiya at anaiya@unityhills.org

Please plan to have all of your decor, photos, memorabilia and food to the church the day before the Celebration of Life, unless otherwise agreed upon, to ensure a smooth flow to your time.
